Abstract: Employee #1 was a mechanic assigned to replace the engine of a 1995 Oldsmobile 98 car. The car was on a concrete floor. Employee #1 used a Pro-lift hydraulic floor jack to elevate the passenger side of the car. While the car was elevated, Employee #1 placed a single jack stand under the right front side of the car. Employee #1 then lowered the hydraulic floor jack so that the car was only supported by the single jack stand. Employee #1 did not chock the wheels of the elevated car. Employee #1 went under the car using a "red crawler" (a four-wheel roller). While Employee #1 was removing equipment from under the car, the car fell from its supporting jack stand. Employee #1 was crushed and killed. The cause of death was compression asphyxia.
