Abstract: On July 24, 2004, an employee of a construction crew installed a French pipe near a road. After the pipe had been laid, an operator of a Caterpillar front-end loader (Model 277) pushed rocks from the road, and pushed and smoothed the dirt on top of the pipe. At the time of the accident, the operator was driving his front-end loader in reverse. While in reverse, the front-end loader struck the employee and ran over him. The vehicle was equipped with a back-up alarm and reverse signal that were operative. The employee was taken to a hospital with fractures to his skull and face. He died a day later from his injuries.
