Abstract: On September 27, 2002, Employee #1, operating the Number 10 extrusion line, was experiencing a problem with the automatic cut off saw. The saw was cutting irregular lengths of product out side the parameters of the client's specs. The supervisor decided to switch to the back up saw, a Dewalt radial arm saw. Employee #1 and the supervisor switched the take off table with the radial arm saw set up. Product was catching on the wood outside of the kerfs. Employee #1 attempted to put some tape to prevent the product from hanging up. Employee #1 did not turn off the radial arm saw when he lifted the product to place the tape on the table. When lifting the product it came in contact with the blade, propelling it. Employee #1's fingers were amputated.
