Abstract: On September 10, 2004, Employee #1 assembled and nailed 2-inch by 4-inch studs into a wall frame in a 3-story townhouse. The frame was lying approximately 2 feet away from an unguarded and unprotected stairway opening. As he lifted the wall frame into place, he stepped backward and fell into the stairway opening. He fell 20 feet, suffered head injuries, and was killed.
