Abstract: At approximately 9:20 a.m. on March 30, 2006, Employee #1 was working as the owner of Nava Roofing & Construction. He was inside a structure, installing drywall while positioned on a ladder that had been placed horizontally between two railings. He was not wearing a harness and lanyard, and was there no other fall protection in place. He fell from the ladder 12 feet, and he was killed. The nature of the injury is not specified in the report. The weather was not a contributing factor, as the work was being done inside on a cool day. The company did not have a plan in place requiring the use of fall protection when workers were at a height of 6 feet or more. There were three employees onsite at the time of the accident, but none were in the area where the fall occurred.
