Abstract: Employee #1 was using a 4 by 8 ft sheet of plywood balanced on the tines of a forklift as a work platform to load a copier through a second-story wall opening onto the forklift. He was kneeling on the sheet of plywood to position the wooden guides for the wheels of the copier when he shifted his weight onto an unsupported part of the plywood. The elevated platform collapsed and Employee #1 fell 9 ft. He suffered a compound fracture of his left fibula and was transported to the hospital. His recovery time was estimated at nine to ten weeks, without complications.
