Abstract: On November 9, 2008, Employee #1 was replacing a light bulb from an 8-foot-high wood stepladder at a hotel. The ladder was being used as a straight ladder with its legs resting on the landing of a wood staircase and its upper portion leaning on a door. The door consisted of open glass and wood construction. As Employee #1, who was 6 feet tall, held the light fixture in his left hand, he ascended the ladder approximately six rungs. When his hands reached within 8 inches of the 13 foot ceiling, the door moved slightly. This caused the ladder to slip and fall down the staircase. Employee #1 fell over the staircase rail to the dining area, about 7.5 feet below the landing, striking his head. Employee #1 rolled to his back and was bleeding from his right temple of the head, as well as had a bruised right arm and shoulder, and fractured vertebrae. Employee #1 was hospitalized at San Francisco General Hospital, where he was treated for his injuries. The accident investigation revealed that the incident was caused from the improper use of the ladder by securing it in such a way that it would not have slipped.
