Abstract: At approximately 12:10 p.m. on November 2, 2007, Employee #1, a laborer with Applied Restoration, Inc., was carrying eight to ten bags containing Primus DS concrete from the second floor down to the first floor. He carried the bags, each of which weighed between 40 and 60 lb, one at a time, using a temporary metal stairway. Employee #1 was carrying the last bag down the stairway and was on the last step when he heard and felt something pop in the lower portion of his back. He continued walking with the bag for about 10 ft until the pain was so severe that he put down the bag. Later, Emergency Services was called and Employee #1 was transported by ambulance to the San Francisco General hospital trauma center. He was admitted and treated for 3 to 5 days, and then sent home to further recover. There were no direct witnesses to the accident.
