Abstract: At approximately 5:23 p.m. on August 16, 2001, Employee #1, a welder, was working on the exterior metal window frame of a hospital building. He used a portable handheld grinder to remove the rust off of a window mount. When he finished, he wanted to turn the grinder off. The grinding wheel fragmented, possibly from an accidental contact with the window frame. A piece of the wheel shot through a magnifier and a poly-carbon lens mounted within his welding helmet and into his right eye. He yelled for help, and his coworkers came to his aid. Employee #1 was hospitalized and lost his eye.
