Abstract: At 3:18 p.m. on June 6, 1999, two employees working for Universal Sign Company were working from a swing scaffold at the site when it fell to the ground. Both employees were not injured when the scaffolding fell because they were wearing an approved safety harnesses and lanyards attached to lifelines. The San Francisco Police Department found two 0.375-inch swedges near the scaffold. It was determined at the site that these swedges were used to connect the wire rope to the parapet clamp. The parapet clamp and wire rope were holding up the scaffolding. The problem with the swedges was that they were 0.375-inches and the wire rope was 0.625-inches in diameter. The swedge did not match the rope. Therefore the wire rope slipped out of the swedges causing the scaffolding to fall.
