Abstract: On March 15, 2005, Employee #1, an apprentice rigger, was informed by a manager of an upcoming crane job. The manager instructed Employee #1 that his assistance in this particular assignment give him the opportunity to gain additional experience toward becoming a rigger and crane operator. Employee #1 notified the manager that he would participate. The crane was set-up and a prepick safety meeting was conducted. During this operation, Employee #1's job was to securely attach straps from the hook of the crane to the base corners of air-conditioning units, which would be lifted to the roof of an adjacent building for installation. There were 12 air-conditioning units in four stacks of three that were placed tightly together at the front of a flatbed trailer. Employee #1 pried the first unit away from the others in order to attach straps to the corners of the air-conditioning units; however, he was only able to access three of the four straps. Unable to access the fourth corner, Employee #1 pull the unit further away from the others and set it down on the flatbed trailer. As Employee #1 was standing on the flatbed trailer, he suggested that the crane operator set the unit on the ground. The crane operator did not agree with the recommendation from Employee #1, deciding to set the unit on the trailer instead. The unit broke free from the stack, with considerable energy and the boom bounced up and down. Employee #1 stepped toward the load to steady the unit, but was unsuccessful and the top-heavy, unbalanced load flipped over and struck him in the head. Employee #1 was hospitalized and treated for a fractured neck.
