Abstract: On January 20, 2003, Employee #1, a batch maker trainee, went to check on the level of rice flour in a hopper. When he observed the flour backing up into the pipe that was feeding the hopper, he used his hands to redistribute the flour in the hopper. While moving the flour with his left hand, his index finger came into contact with a paddle like rotary valve located within the top of the hopper. His index finger was amputated. Medical assistance was called and the Woodland Fire Department responded and transported Employee #1 to Woodland Memorial Hospital.
