Abstract: At approximately 5:00 p.m. on May 4, 1997, Employee #1, a carpenter, was working on a remodeling job. He was using a table saw with the blade set 2 in. above the table to make a V- notch in a 1/4 in. thick piece of trim material . His left hand struck against the unguarded saw blade, amputating three fingers at mid-knuckle. Employee #1 could not recall exactly how his hand came in contact with the blade.
