Abstract: Employee #1, working for McDonald's as a lobby worker, was walking up the stairs after picking up the tray liners from the stock room when she saw a box of ketchup misaligned in the conveyor track. When she stopped and straightened the box, her hand became caught in the running chain and sprocket of the conveyor. She sustained an amputation of her left ring and pinky fingers. The injury did not require hospitalization. After further investigation, it was determined that the moving chain and sprocket drive on the conveyor was not guarded.
