Abstract: At approximately 8:15 a.m. on July 16, 2004, an employee was tending a conveyor line of 'Top Ramen' noodle packets at Nissin Foods located in Gardena, CA. It was his function to check the conveyor feed and take off packets blocking and creating a snag in the line. Snags occurred frequently when the product entered another conveyor. The employee took off packets of product and placed them into a plastic bin. The conveyor had a sprocket and sprocket chain drive in close proximity to where he worked. He was unaware that the sprocket and chain had no cover on this particular shift, because it was guarded the previous day while he worked there. When he tried to catch packets of products that fell off the line, his cotton gloves got entangled in the chain and a finger got caught in the sprocket. A finger was amputated, but he did not require hospitalization.
