Abstract: On April 24, 2008, an employee was employed at Fishking Processors, Inc. as a sanitation employee through a temporary labor agency. His job duties consisted of cleaning several types of machines. On the day of the incident, the employee was cleaning the blender. As he was manually lowering the discharged door with his left hand, his fingers made contact with the rotating auger. The auger sheared off the tips of his fingers. As a result of the incident, he sustained finger tips amputation. He was not hospitalized. Blender was manufactured by the Blentech in Rohnert Park, California, Model Number DM1542.F, Serial Number. 92450 480 Volts, 30 amps, 3 phase. Investigation revealed that the equipment's energy sources were not deenergized and locked out before cleaning.
