Abstract: The site was a concrete block building that had a structural steel equipment platform being constructed against the east wall. It was 13 feet high, and an approximately 5 by 16 feet square. The decking was laid out on support beams, but was not supported by the ledger. A coworker was on one of the cross beams preparing to weld the decking. Employee #1 normally wears contact lenses, however on this day he switched to glasses, and did not notice that the decking was not on the edge of the ledger. The unsupported edge of the decking bent downward and he fell 13 feet. As he was falling, he tried to grab a piece of the rebar extended from the edge but was unable to. Employee #1 tore the supporting tissue in his shoulder, and he fractured his ankle on impact with the concrete below. He was hospitalized for his injuries.
