Abstract: On November 19, 2001, Employee #1, of Gateway Energy in Corona, CA, was the supervisor for the electrical installation at a new office building. At the time of accident he was tracing a conduit that was running through the framework overhead by visually following it. As he turned a corner, he noticed another worker inside a duct overhead; this distracted him and he fixed his gaze upward. As a result, Employee #1 failed to notice an 4.5 ft by 6.5 ft open mechanical shaft. He fell 15 ft through the unguarded opening, landing on a concrete surface. He fractured both wrists and shattered his elbow.
