Abstract: On May 11, 2001, Employee #1 was working on a building remodeling project. He put up an extension ladder to access an I-beam located 13 ft from the concrete floor, where he intended to attach a negative lead for his welding machine. When, at a subsequent time, Employee #1 was coming down the ladder, he missed a rung with his foot and fell approximately 10 ft, breaking two ribs and an ankle. Employee #1 was hospitalized. He was employed as a welder for 9 months before the accident.
