Abstract: At approximately 2:25 p.m. on April 25, 2001, Employee #1 fractured both hands when he fell twenty feet through an unguarded hole while he was sweeping an area under construction with several large floor openings, the size of small car. Apparently, the barricades used to secure the area had been removed to allow cleaning of the area. Employee #1 was sweeping the area just before the accident occurred with two coworkers. The Forman was operating a forklift in the area of the accident. Employee #1 fell approximately 20 feet to the basement level of the building. Employee #1 fractured both hands. His employer failed to provide a guard around the hole to prevent falls. Employee #1 went to a local hospital for treatment.
