Abstract: At 6:00 p.m., on July 19, 2000, Employee #1 was working on the Makor machine when the belt on the small pulley came out. He stopped the machine by pushing the button, put the belt back on the small pulley, and re-started the machine. The belt came off again, and this time Employee #1 tried to replace it onto the pulley without stopping the machine. His right middle finger became caught between the pulley and the belt, and was amputated at the middle of the second joint. He was transported by Gardena Fire Department paramedics to Gardena (CA) Memorial Hospital, where he was treated and released at approximately 7:45 p.m. on July 20, 2000. When Employee #1 tried to reposition the pulley the second time, the safety door to the chamber was open, but the machine was operating and the pulleys were rotating.
