Abstract: Employee #1 is the quality control supervisor of the cook room of a meat packing company that processes and cooks ground meat. On October 19, 1999, while working his normally assigned duties in the packaging area of the cook room, Employee #1 noticed that the gear and chain drive was dirty. He then removed the Plexiglass guard over the drive, and began to clean it with a rag. While doing so, the rag got caught in the chain which dragged his finger into it and amputated the finger tip. The company has a well-devised and enforced safety program.
