Abstract: At approximately 12:55 a.m. on April 16, 2004, an employee, a drilling foreman was supervising a job task that involved removing a 3-foot by 6-foot steel trench plate that was being used as a temporary shaft cover over a 30-inch by 50-feet deep shaft. The employee signaled the crane operator to lift the plate and walked or stepped forward and fell feet first down the shaft. The employee was able to place a "doughnut" collar on himself and the fire department personnel manually hoisted him out of the shaft. After an evaluation by paramedics, the employee was taken to the medical center, where he was admitted and treated for a broker right ankle, dislocated shoulders, three fractured discs, a scratched cornea and lacerations to his hands and left cheek. He was hospitalized for seven days.
