Abstract: At approximately 9:00 a.m. on February 6, 2004, Employee #1, a foreman, and a coworker were installing a fence along the top of the brick wall at the north end of the courtyard in a San Bernardino County Correctional Facility. Employee #1 and the coworker were in the elevated basket of a rented scissor lift, approximately 14 ft above the concrete surface of a basketball court. Employee #1 was operating a scissor lift, which was moving to the west along the north wall of the courtyard toward a 15-ft by 15-ft floor opening. The scissor lift capsized when it made contact with the floor opening. Employee #1 fell approximately 14 ft from the scissor lift basket when the basket fell to the ground. Employee #1 sustained fractures of multiple body parts and was transported to Loma Linda University Medical Center, where he underwent a surgery. He was hospitalized for 11 days.
