Abstract: At approximately 11:20 a.m. on November 14, 2003, Employee #1, a cleaning worker, was cleaning a rain gutter on a building, approximately 15 to 16 ft high. He was using a Hydro Pro pressure washer, Model 4G35H, at 2500 psi. He was standing on a 12-ft Louisville fiberglass step ladder, Model FS1012. He had one hand on the pressure washer trigger and the other hand on the wand, standing freely on the stepladder. The pressure washer was exerting force on his hands, making it difficult for him to handle it properly. This made the ladder unstable and the ladder tipped over. He fell to the concrete floor and fractured his right collarbone and right temporal bone. Employee #1 was hospitalized.
