Abstract: At approximately 3:29 p.m. on December 9, 2003, Employee #1, a service technician of the Employers Resources Management, Incorporated employment agency, was working for 24 Hour Fire Protection, Incorporated. Employee #1 was primarily engaged in repair, maintenance and certification of fire suppression systems of the commercial/industrial accounts. According to a witness coworker, at approximately 3:00 p.m., both Employee #1 and the coworker parked their service trucks (trucks Numbers 2 & 3) at the north curbside of the 2200 block of East Philadelphia Street in Ontario, California. They were preparing to perform repairs on fire suppression equipment (i.e., PI valve) located approximately 17 feet away from the north curbside. During repairs, Employee #1 was picking tools from service truck Number 3 when a west-bound flat bed trailer truck struck against parked service truck Number 2, pinning Employee #1 between service trucks Numbers 2 and 3. He sustained multiple blunt force injuries of his body. He was taken to Chino Valley Medical Center, where he was pronounced dead at 4:14 p.m.
