Abstract: At approximately 6:25 p.m. on December 29, 2003, an employee was operating a 480v, SMETCO, Inc., pallet sorting machine with a Baldor 3 phase motor. The machine was making noises; the employee called maintenance. The maintenance man did not lockout or tagout the machine and removed the sprocket chain guard, which was 40-in. above the floor. During the troubleshooting process, the maintenance man was called to look at another piece of machinery and left the employee standing by. The employee turned the machine on and started the operation moving. Two pallets came together on the conveyor. The employee reached across to move the pallets, at the same time, he placed his gloved left hand on the chain. The employee's index finger was caught in the moving sprocket chain drive. The employee sustained a de-gloving and partial amputation of the distal phalanx of his left index finger. He was treated and released from hospital.
