Abstract: At approximately 11 a.m. on November 12, 2002, Employee #1, a journeyman carpenter, was walking on the roof of a 12,880 square foot tilt-up commercial building under construction. Employee #1 was walking on a section between a panelized roof section and a ledger that was a work in progress. A 6 ft by 8 ft section of the fill-in section gave way under Employee #1's weight, and he fell 25 feet to the floor. The top flange of the sub-purlin hanger (Simpson Strong-Tie F46) supporting the 4 in by 6 in by 6 ft support was not nailed to the ledger. Employee #1 did not connect his body harness to the lanyard and clip. Employee #1 was initially hospitalized at Inland Valley Hospital in Murrieta, California with multiple fractures to his left heel, compression fractures to L3 and L4 vertebrae, and a compound fracture to his left wrist. Employee #1 was subsequently transferred to Loma Linda University Medical Center for corrective surgery and was hospitalized for 10 days.
