Abstract: At approximately 12:31 a.m. on November 18, 2001, Employee #1, a 49-year-old male, with W.T. Lighting Corporation, was lying on his back on top of a refrigerated metal beer box that was approximately 6.5-feet from the tile floor and contained alcohol products. Employee #1 was to replace and install 277-volt electronic ballasts for the fluorescents lights that were above the metal beer box. The supervisor had spoke to Employee #1 earlier and then came back to check on him and found him lying still with smoke coming from his body and his left index finger. The supervisor noticed two black wires hanging from the ceiling and that the wires were stuck to Employee #1 left index finger. The supervisor used a pair of wire cutters to cut the wires from Employee #1 fingers. The supervisor then removed Employee #1 from the refrigerated beer box and started mouth to mouth resuscitation. Employee #1 was transported to San Bernardino Community Hospital where he was pronounced dead at 1:10 a.m. Employee #1 was not wearing insulated gloves as required when working with voltages in excess of 250-volts to ground.
