Abstract: At approximately 9:30 a.m. on June 18, 2001, Employee #1, a temporary employee for Authorized Personnel Services, Incorporated, lost his middle and index fingers while he was making steel clips on an unguarded "CHICAGO" Press Brake at the JLMC, Incorporated. The Model Number was 68-B. He placed a 3.5 inch by 3.25 inch steel flat piece. He inadvertently pressed the foot pedal and his right hand became caught in the point of operation causing amputation of his right middle and index fingers. The press brake was not equipped with a safety device or guard at the point of operation. Employee #1 was taken to U.S. Healthworks in Ontario, California for treatment. He had surgery the next day at Chino Valley Medical Center in Chino, California.
