Abstract: At approximately 10:15 a.m. on March 22, 2000, an employee was in the process of changing dies on a Scotchman-Ironworker, Model number 6509-24FF punch press. The press was located in the southwest part of the production area. During the changing process, the power was left on and for some unknown reason, the power press activated at which time the tip of the punch die located in the ram broke, hitting the right lens of the employee's eye glasses and shattered it. The projectile of the broken piece hit the eye lid, causing a serious injury resulting in the loss of eye sight. The eye glasses were not ANSI Z87 approved. The employee was taken to the Loma Linda University Medical Center for treatment and surgery was performed the same day.
