Abstract: On February 23, 1999, Employee #1 suffered first-and second-degree burns to his face and arms while refilling a propane tank on a motor home. Upon arriving on the scene, the first fire unit observed a class C motor home on fire with a propane tank refilling still attached. Investigators determined that the refrigerator pilot light apparently was still on. This pilot light, which was located right above the tank, was the source of the ignition. The motor home was destroyed. Employee #1 failed to ensure that all sources of ignition, including the pilot light had been turned off. He was transported to Desert Hospital, located in Palm Springs, Florida, for treatments for burns to his face and arms. He remained in the hospital for one week.
