Abstract: On February 23, 1999, Employee #1 lost right arm above his elbow, when his arm became caught between a motorized trunion wheel and a steel pipe. He was welding and grinding a 23,000-pound, 40-foot-long steel pipe. The diameter of the pipe was 109 inches. He rotated the pipe downward using a hand-held, pendent control. As the pipe reached the area to be worked, he inserted a shop-made steel wedge in between the pipe and the trunion wheel. At that time, Employee #1's arm became caught between the pipe and the wheel. The machine did not have any protection guards. He was transported to Loma Linda University Hospital for treatments for his amputation.
