Abstract: At approximately 6:45 a.m. on March 26, 1997, Employee #1, a carpenter, was working on a roof structure. He was removing the cover from a previously installed air conditioning duct hole when he fell 32 ft 4 in. through the roof opening. Employee #1 was killed. The air conditioning duct hole had been securely covered with plywood and marked "danger open." The reason for Employee #1 removing the cover was never determined.
