Abstract: On March 9, 2007, Employee #1, a machine operator, was operating a Baler, manufactured by U.S. Wire Systems Woodbridge, Ill, Model Number 331 Serial Number 3083. This baler ties bundles of cardboard together with wire. The bundles are approximately 6 ft long by 4.5 ft wide by 4 ft tall. Employee #1 used an approximate 2-in. by 4-in. sponge to clean excess oil off the gears each morning. These gears pull wire through the machine for tying the bundles. He indicated that there was too much oil on the gears and the wire would slip off causing the machine to not work properly. He had his right hand on the sponge to clean the gears while he advanced the gears with his left hand by operating the activation button. The gears caught his hand and amputated his right middle finger.
