Abstract: On May 12, 2006 Employee #1 was constructing false work for a bridge. The false work was built with horizontal cross members every 4 ft and there were 7 levels. The first set of cross members was 6 ft from a concrete structure at the bottom. Employee #1 was tying off to the 8-in. wide walers while moving across them. When ascending or descending cross members, workers were not tied off since to tie off at a lower or upper level you first have to be at that level. Employee #1 had drilled holes at the northeast corner at the top and was climbing down to a lower level. At the fourth row of cross members from the ground he grabbed onto an unsecured 4-in. by 8-in. cross member and it gave way causing him to fall 18 ft to a concrete surface. Employee #1 suffered a fractured right ankle and a dislocated left ankle. Employee #1 was hospitalized.
