Abstract: On January 24, 2005, Employee #1 was assigned the task of towing a motor home from a residential site to a repair shop. It was a 40-ft long 2005 model motor home, which had starting problem. Before towing this type of vehicle, the driveline shaft is removed to avoid burn out of the transmission during the tow. Employee #1 first hooked up the disabled motor home to his tow truck and pulled it slightly forward. Then Employee #1 crawled under the motor home and started to disconnect the drive shaft from the yoke on the rear end of the shaft. There were four bolts connecting the shaft to the yoke. As soon Employee #1 removed two bolts between the shaft and the yoke, the shaft started to spin rapidly. It appears that the remaining two bolts broke off due to the excessive torque built up in the shaft. The spinning shaft end fell on Employee #1 and injured his right arm and chest area. Employee #1 suffered multiple fractures to his right arm and sternum. Employee #1 was hospitalized for four days.
