Abstract: At approximately 4:00 pm, on November 8, 2004, Employee #1, a concrete subcontractor, was welding underneath an 18-foot long, 8-foot wide roll up detached flatbed. The employee had elevated the front end of the flatbed 5-feet using a forklift, and had added a circular steel pipe on each side of the front of the flatbed for additional support. Each pipe was approximately 5-feet long and 1-inch diameter. The rear end of the flatbed was at ground level and stationed with a 2x4 block of wood placed behind each of the rear wheels to act as wheel stops. Employee #1 was kneeling on his left knee with his right knee bent while welding, when the load began to shift.
