Abstract: On June 25, 2004, Employees #1 and a coworker were painting industrial equipment, which includes a chip vacuum tower that is used for collecting metal chips from a large machine shop. The tower consists of industrial vacuum equipment located on top of a metal structure, which is about 27 ft high. The tower rises another 10 ft above the 27 ft level. Below this structure, there is a hopper used for dumping metal chips. The top of the hopper measures approximately 12 ft by 10 ft and has four vents on each corner. Each of these vents measure 48 in. by 34 in. and they are covered with a wire mesh and a sheet metal hood. Employee #1 was working on top of the hopper when he accidentally stepped onto a vent and fell through the hopper into the chip collecting bin. According to the coworker, the vent that Employee #1 fell through did not have a sheet metal hood; it only had a wire-mesh cover. The employee fell approximately 27 feet into the bin and suffered head and facial injuries. Employee #1 was hospitalized for three days.
