Abstract: At approximately 7:28 a.m. on August 5, 2003, Employee #1, a full-time carpenter, was standing on a second story floor awaiting a delivery of panels. The plywood panels were 4-ft wide by 10-ft long and 0.75-in.-thick. A coworker operating the forklift would raise the panels up, even with the second story window. Employee #1 would pull the panels off of the forklift and through the window. There were 60-to-70 unsecured, banded panels on the forks. Employee #1 stepped through the stud wall onto the top plywood panel to break the bands. The load shifted causing Employee #1 to fall approximately 8 ft to the ground. Employee #1 suffered spinal cord injuries and was hospitalized.
