Abstract: On November 21, 2003, Employer #1 was working for an employer that was engaged in the installation of ATM machines at various customer sites. Employee #1 and some coworkers noticed a dead light bulb in one of the fixtures located about 30 ft above the warehouse floor at the employer's facility. The light fixture was hanging from the ceiling with a chain. Employee #1 volunteered to replace the dead light bulb. His immediate supervisor and a coworker offered to help him. The light fixture was located above a 3-tier storage rack that had a top shelf that was 16 ft above the floor. Employee #1's warehouse supervisor elevated him and the coworker to the top shelf with a forklift. A standard 48-in. by 42-in.wooden pallet was used for elevating the employees. The employees setup a 10 ft stepladder on the shelf. Employee #1 climbed up the ladder to unscrew the bulb. As he was holding the light fixture in both hands, he lost his balance and fell backward down to the floor, about 23 ft below the point where he was standing on the ladder. Employee #1 could not remember if the light bulb broke or not, but witnesses stated that the bulb burst in his hand as he was trying to unscrew it. Before falling to the floor, Employee # 1 hit a wooden pallet sitting on an intermediate shelf about 4 ft below the top shelf. The stepladder was set up on the top shelf, which was 96 in. The base of the A-frame stepladder opens to a width of 66 in. The ladder was setup across of the 96 in width of the shelf, which left about 15 in between the base of the climbing side of the A-frame ladder and the unprotected edge of the 16-ft high shelf. There was no fall protection provided during this entire operation. Employee #1 suffered multiple fractures, including neck and spinal injuries. Employee #1 was hospitalized and treated for his injuries.
