Abstract: At 8:30 a.m. on December 9, 2002, Employee #1, a full-time pump truck operator, was troubleshooting the Olin p-gravel pump truck, Model PBS1ES, he was using to pour a foundation for a residential property being constructed in San Juan Capistrano, California. The pump was clogged, so he shut the pump down. He did not find any clumps when he walked the hose line. He removed the reducer elbow clamp and hit the reducer elbow with a hammer. A built up pressure in the chamber caused the reducer elbow to blow off and hit his left arm. He sustained multiple fractures to the left arm and was hospitalized. Employee #1 did not follow the procedural steps of reducing the pressure and releasing the pre-charge from the surge chamber prior to removing the elbow reducer clamp.
