Abstract: Employee #1 was a union trade show decorator/installer working for convention booth installing company with components for the booths stored at its warehouse. He was trained to operate industrial trucks used to move material. He was refilling a propane fuel container for an industrial truck when an unknown ignition source caused the propane to explode. Employee #1 suffered second-degree and third degree burns to his arms and face. He was hospitalization for 12 days.
