Abstract: At about 9:00 a.m. on May 27, 1998, Employee #1, who worked as a resin departmental supervisor, noticed that one of the belts on a double belt pulley had just broken. In order to replace the broken belt, Employee #1 turned the machine's power off and removed the guard on the belt-and-pulley. He tried to pull out the broken belt with his right hand while the pulley was still coasting on the other belt. His right ring finger was amputated. Employee #1 sustained serious lacerations to his right index finger and right middle finger. Employee #1 was hospitalized and treated at the Fullerton-Placentia Medical Center.
