Abstract: At approximately 8:05 a.m. on September 23, 2008, Employee #1 and a coworker were loading an ice cream-freezer (Soft Serve Freezer - Model Number 338) on the 'lift table' of delivery truck. The freezer was sitting on a delivery cart. The coworker was pushing this freezer from the top towards the lift-table. The freezer weighed approximately 465 pounds. Employee #1 noticed that freezer was not moving forward, so he attempted to lift the freezer from the bottom. The coworker did not see Employee #1's hand while he was lifting. Coworker pushed the freezer again and Employee #1's finger got stuck inside the back-panel which sliced the first tip of his finger (ring-finger of the left hand). Employee #1 was taken to the hospital where his finger was amputated. Employee #1 was treated but was not hospitalized.
