Abstract: Two workers were cutting steel I-beams measuring twelve inches square by forty feet long and weighing about fifty three pounds per foot, which were resting on the forks of a rough terrain forklift when Employee #1, a foreman, told them to stop. Employee #1 had them set the beams on the ground and instructed the forklift operator to report to another area. The forklift operator was unable to leave as the access road was blocked by other equipment. Employee #1 ordered the operator to drive over the beams and the operator, hesitating at first, complied and drove over the beams. The tires kicked up one of the beams which landed on Employee #1's foot, amputating three of his toes. He was hospitalized.
