Abstract: At approximately 8:20 a.m. on September 24, 2003, Employee #1 was working in a warehouse and was retrieving materials from their stored location and delivering them to shipping and receiving. Three days prior to the day of the accident, the order picker used to retrieve items from the shelves was not functioning properly and was removed from service. Commonly, when the order picker was out of service, employees would mount a metal frame and platform to the forklift and elevate themselves to the level required to retrieve materials from the pallet rack in the warehouse. However, the guardrails on the platform were not at the height as prescribed applicable standards and the platform did not have a means to properly secure the cage to the forks. In addition, the combination of the forklift and cage did not have the capability to reach the highest level of the pallet rack. To gain additional height, the cage was mounted atop three pallets with the lowest pallet engaged with the forks. The pallets and the platform were shrink wrapped together to form a single unit. On the day of the accident, a coworker was lifting Employee #1 to the top level of the pallet rack to retrieve items to be shipped. The coworker tilted the forks and Employee #1 lost his balance and fell with the cage to the ground. He sustained broken ribs, a punctured lung, and additional back and leg injuries and was hospitalized for 3 days at Henry Mayo Newhall Memorial Center.
