Abstract: At approximately 12:00 p.m. on March 21, 2003, Employee #1 covered the ground with plastic to protect the landscape work, which was already done. He put up an extension Davidson fiberglass ladder, 3000-pound capacity, along the garage wall. The ladder was extended 12 ft and about 3 to 4 ft above the garage's roof. He climbed up on ladder with his tools, hammer, knife, and tape. He was approximately 7 ft high when the ladder slipped out. The ladder was not tied, blocked or secured at its foot. He was hospitalized with four broken ribs.
