Abstract: At approximately 10:00 a.m. on January 15, 2003, Employee #1 was working with a crew of maintenance workers installing new lockers in the employee locker room at the City of Los Angeles, West Valley sanitation yard. The sanitation yard is located at 8840 Vanalden Avenue in Northridge, California. The crew was using a nail setter and lost track of it. The rest of the crew decided to break for lunch while Employee #1 stayed behind to look for the nail setter. First, Employee #1 checked the insides of the lockers for the tool, but did not find it. He then decided to check on the top of the lockers. So he opened locker 267, which was attached to two other lockers, and tried to hoist himself up to the top of the lockers by using the interior hanger bar of the locker. The weight of the combined weight of the three lockers was 200 pounds. This set of lockers had not been bolted or secured to the pedestal and they tipped over onto Employee #1. The lockers hit him in the head. Other workers nearby picked the lockers up and off of the employee, and informed the supervisor of the accident. Employee #1 was told to sit back and let rescue personnel to check him out. The employee was taken to Northridge Medical Center, where he was treated for a fractured right eye socket. Employee #1 was hospitalized.
