Abstract: At approximately 9:45 a.m. on May 29, 2002, Employee #1 was working on the press brake machine (DURMAZLAR, Type: HAP 30120; Year: 1999; Capacity: 132 US TON; Serial Number: MASCH-NR 7042992782; Stroke: 155). Employee #1 had been worked with this machine about a week, and was trained by his supervisor. Employee #1's right index finger was caught between the point of operation when he tried to get and put the sheet of metal back to its position. There were no witnesses at the scene. The press brake machine did not have any safety devices or guard for the point of operation. Employee #1 suffered the amputation of his right index finger to the first knuckle. Employee #1 was hospitalized.
